The doula intake form is built to assist doulas in asking questions and becoming fully versed in the needs, wants, and desires of the expectant mother during her pregnancy and labor. This intake form features three pages of information organized by topic to assist in the initial assessment.

Those topics include the following:

• About Client – Basic information about your client.
• About Care Provider – Contact information for primary care provider and additional classes or training client may already have.
• Health History – A summary of health related issues for the client.
• Your Pregnancy and Birth – A questionnaire-type list used for assessing current pregnancy and expectation of the client for this pregnancy.
• Doula Birth Care Plan – A list of needs identification areas that a doula will plan to address in providing service to the client.
